Implantation d’un triage infirmier pour évaluer l’aptitude à la transition dans une clinique de suivi à long terme en oncologie pédiatrique durant la pandémie de COVID-19 : obstacles et facilitateurs

Abstract


Cette étude répond à deux objectifs : décrire l’aptitude à la transition vers les soins aux adultes chez les mineurs survivants d’un cancer et leurs objectifs personnels à partir d’un échantillon clinique de taille limitée; et étudier le point de vue des professionnels de la santé sur l’utilisation clinique par les infirmières du Questionnaire sur l’évaluation de l’aptitude à la transition (TRAQ) et sur son interruption dans une clinique de suivi à long terme en oncologie pédiatrique. Le questionnaire a été remis à de jeunes survivants du cancer (n=7) dans un hôpital pédiatrique tertiaire, et des entrevues approfondies ont été menées auprès de professionnels de la santé (n=3) d’une clinique de suivi à long terme en oncologie pédiatrique. Les données qualitatives ont révélé des obstacles, des facilitateurs et des stratégies pour l’implantation du TRAQ en lien avec l’outil, les professionnels de la santé, les adolescents, et le contexte clinique. La compréhension de ces obstacles et de ces facilitateurs permettra l’élaboration d’un plan d’implantation du questionnaire en contexte clinique. L’analyse des données révèle des degrés variables d’aptitude à la transition parmi les patients ainsi que des thèmes récurrents dans leurs objectifs, ce qui confirme la nécessité d’une préparation à la transition vers les soins aux adultes pour les jeunes patients.
