Tirer parti de la coproduction pour la revue systématique des données nécessaires à la mise à jour des guides de pratique COSTaRS (Projet pancanadien de triage des symptômes et d’aide à distance en oncologie)

Abstract


Nous avons eu recours à la coproduction pour : 1) réaliser une revue systématique visant l’actualisation des données sur l’évaluation, le triage et la gestion dans 17 guides de pratique de triage des symptômes et d’aide à distance en oncologie (projet COSTaRS); et 2) évaluer l’approche de coproduction utilisée par notre équipe pour contribuer aux données sur l’importance de faire participer des personnes qui utilisent des connaissances (patients, proches aidants, infirmières, gestionnaires et responsables des politiques qui sont Pouutilisateurs de connaissances).

Parmi les 17 618 articles trouvés depuis la dernière mise à jour publiée en 2020, 175 (dont 124 guides de pratique clinique et 51 revues systématiques incluant des méta-analyses) étaient admissibles. Les conclusions ont été utilisées pour actualiser les 17 guides de pratique COSTaRS et en créer un nouveau sur la dysphagie. Les données provenant de 32 sources obsolètes ont été supprimées. Les guides de pratique COSTaRS a nécessité quelques mises à jour sur l’évaluation, le triage, les médicaments et la prise en charge de soi afin de refléter les nouvelles données identifiées. 

Un sondage préliminaire a permis d’évaluer l’expérience des membres de notre équipe avec les guides de pratique COSTaRS, l’expérience avec la réalisation de revues systématiques et leur niveau d’implication préféré pour chaque étape de la revue systématique. Au total, 14 membres de l’équipe sur 15 (93,3 %) ont répondu à ce sondage. Une enquête de suivi utilisant notamment l’Échelle d’engagement des patients dans la recherche (PEIRS-22) a été réalisée par 12 membres de l’équipe (2 patients ou proches aidants de patients, 5 chercheurs et 5 infirmières), et montre un niveau d’engagement extrêmement élevé pour chaque sous-score et le score total PEIRS-22.

Les commentaires des participants sur le processus dénotent qu’ils se sont sentis valorisés, traités comme des partenaires à parts égales et impliqués dans les mises à jour régulières du projet. Les participants ont aimé pouvoir sélectionner des niveaux d’engagement individuels et selon eux, l’inclusion de différentes perspectives a amélioré la richesse du produit final. Bien que leur participation ait été influencée par les contraintes de temps liées à la charge de travail, un fort sentiment d’appartenance relativement au produit final a été identifié comme un élément favorable au processus de coproduction.
